1. Synthesize knowledge from liberal arts and sciences and nursing science in understanding global perspectives, stimulating critical thinking, and using current technologies.
1a. Demonstrate use of critical thinking during creation and implementation of project as expressed in journal entries during weeks 2, 4, and 6.
1b. Review and become familiar with fall policy and procedure of ContinueCare Hospital by week 2.
2. Make sound decisions in advocating for client in designing, coordinating and providing culturally sensitive population based care.
2a. Evaluate patient call system with use of chair/bed alarms by week 3.
2b. Define the 4 P’s used with hourly rounding in a way that all nurses are familiar by week 2.
2c. Evaluate current fall risk assessment tool by week 3.
3. Provide compassionate and appropriate direct and indirect nursing care in managing illness and disease in individuals, families, and communities in a variety of settings.
3a. Attend daily “flash” meetings with leadership team to evaluate number of falls during current week during weeks 3 thru 5.
3b. Record in weekly journals the observations of frequency of current nursing rounds and current number of falls per week in weeks 2,4, and 6.
4. Communicate, collaborate, and negotiate in an effective manner.
4a. Collaborate with preceptor to prepare for course by week 1.
4b. Collaborate with DQM and CNO on revision of fall policy by week 3.
4c. Communicate the changes to fall policy and implementation of hourly rounding in town hall forum by week 6.
5. Exhibit professional values and behaviors as affirmed by the professional code of ethics and legal statues.
5a. Create a teaching plan for CCH nursing staff that is in compliance with CIHQ regulations by week 4. (Centers for Improvement of Healthcare Quality).
5b. Apply the ANA code of ethics while researching and preparing this project.
6. Employ principles of teaching and learning to promote healthy lifestyles through health promotion, risk reduction, and disease prevention education.
6a. Discuss with nurses how they are assessing patients for fall risk and use of assessment tool in week 3.
6b. Discuss with nurses’ frequency of visualizing patient each shift and questions asked while in room in week 4.
