Locate a funding agency and a possible grant opportunity related to diabetes .  Utilizing information with proposal statement bellow, create a grant or program proposal that addresses or provides a solution to an aspect of the problem.  Use the POARE Model attached bellow. Create a minimum of one goal and two objectives.  Complete all other components of the provided template.

“It is very evident that the lack of education in the relationship between the diabetes and periodontal disease, is very concerning and is a big threat for the future generations. This proposal is to create a program for an interprofessional relationship between a dental hygienist and medical office. The proposal presents a solution to have a registered dental hygienist present at the medical center to screen patients during initial medical visit and provide educational tools in person or via telehealth to better understand the connection between diabetes and oral health. With early screenings and proper education is for the population to understand the serious implication and increase better oral health outcome for future generations”.
