Case Study
You are an intern in a community mental health center in a large urban area where significant number of the people are living in poverty. You are assigned by your supervisor a family of a single mother and her 4-year-old male son for initial assessment. The son is presenting as hyperactive and shows some slowness in development that is age appropriate. In initial home visit you notice that the child is eating pieces of paint from the wall and the back of the closet. The mother is trying to control it but he is still able to find corners where he can do so.

Social Worker: Hello X, my name is X, and I am your son’s assigned social worker. Thank you for the warm welcome. It is nice to see you and your son. When working together, I want to let you know that you have the freedom to talk and contest or appeal any decision. Any information that you share remains private and confidential unless it poses a risk to your son. Please feel free to stop me at any point and ask me any questions you may have.
Client: Thank you so much for visiting us today. It has been a while since a health professional has visited us. I have to handle him (turns to look at her child, peeling paints from the wall and eating) because he is very active. I must remind him about his behavior and actions multiple times per day. Some of his teachers and classmates think my son is odd. It is becoming very overwhelming for him at school. To be honest with you, it’s becoming very overwhelming for me as well, as he is my only child, and I am trying my best to support him.
Social Worker: (with body facing client and maintaining eye contact with client) Why do you feel that his teachers and classmates think your son is odd?
Client: Well, he eats paint, and he is somewhat hyperactive. My son also easily forgets things and has limited attention. He is uncooperative much of the time, which has been such a concern. Additionally, to make sure he does his homework, I must be always with him.
Social Worker: What is the greatest worry you have about him?
Client: I am concerned about him eating paint. That cannot be good for him. I have tried to make him stop, but I cannot. If I try to stop him, he gets ag
