Expected Components of Community Assessment
- Introduction – Catches the reader’s attention with interesting facts and supporting sources. Includes the purpose of the paper.
- Community Data – Community identified and briefly described
- Demographic and epidemiological data – A range of demographic and epidemiological data for your community examined by census reports, vital statistic reports, city records, and other agency sources and compared to state and national data.
- Windshield survey – Community assessed by doing an informal windshield survey: Findings identified things such as types of housing, schools, churches, health care providers, and environmental or safety hazards. Findings relate to identified problem.
- Problem in relation to Healthy People 2030 – Assessment data used to identify the problem. Rational provided for your choice and related to one of the Healthy People objectives. Supportive data provided to validate this is a problem in your community.
- Historical significance – Historical significance addressed. Trends considered in the problem over time and any significant events that may have had an impact on the problem.
- Summary – Includes a statement about the problem, the population at risk for this problem and the major direct or indirect factors that contribute to this problem.
