Week 1

You began the week by exploring the foundations of population health and how change is dominating population health management. You considered how the changing healthcare landscape is placing more emphasis on population health, and you examined the work that is underway to align efforts and resources to enhance the impact on outcomes. You also investigated culturally and linguistically appropriate healthcare and appraised your organization for the provision of these services. What’s more, you utilized an interactive population map to develop a culturagram for a selected population, an intervention to reduce health disparities in this population, and relevant CLAS standards.

Week 2

 Epidemiologic principles and measurements were the basis of your exploration this week. You discovered that epidemiology is about identifying associations between exposures and outcomes. You learned that successes in treating and preventing disease have shifted emphasis to understanding the mechanisms underlying disease and its prevention. A comprehensive assessment of risk factors is now considered foundational in improving population health outcomes. The week culminated in an opportunity for you to apply epidemiological measurements to one of the eight national practice problems.

Week 3

This part of your journey focused on how surveillance and big data have arrived at the prefect time to facilitate healthcare reform. Rather than increasing services to meet demand, healthcare organizations are now using data to improve population health and reduce service levels. Although there are opponents to this approach, many believe this shift not only will make an immediate impact on outcomes, but also will play a vital role in the evolution of the healthcare industry as a whole. At the forefront of this change is the practice scholar as the expert in the translation of evidence.

Week 4

This part of your journey focused on analyzing levels of prevention. You’ve probably realized that as you explored these concepts, you’re already familiar with them. Now you understand them as they relate to improving outcomes for populations. You also investigated population health program interventions and used the population health map to apply the week’s concepts as they relate to a selected population.

Week 5

Effective measurement provides the evidence upon which to compare and improve interventions, share best practices, secure funding and community support, and meet goals and objectives. Without monitoring and evaluation, organizations lack the evidence to support the credibility of interventions.

Week 6

This week was all about emergency preparedness and management. You gained insight into the value and importance of the practice scholar in responding to crisis and organizational challenge.  We all agree that the DNP scholar can contribute to disaster preparedness in all phases of planning and response.

• In and after a disaster, what ethical questions arise regarding the fair allocation of relief funds to help with recovery?
• What do you value most about your learning this week?

Week 7

You discovered how social media and marketing are transforming health campaigns and outcomes. Were you surprised to learn that one of the biggest popular culture trends is turning out to be revolutionary in affecting health outcomes? Or that humor and other engaging campaigns are often more effective than educational ones? I was! Not only are these methods more effective but also they are more cost-effective and sustainable. Health professionals and organizations must recognize society’s ever-increasing use of social media tools, as well as the rewards and challenges associated with using them.

• Why is causation so difficult to prove and how does it define outcomes?
