Mr. Robert is a 79 -year-old gentle man is a retired Paper Mill Employee. His current medical diagnoses include Uncontrolled Diabetes Mellitus type 11, and Coronary Artery Disease. When you consider Mr. Robert’s health risks, you also want to consider his Health Literacy Needs when developing learning needs assessment, goals, and strategies.
Write a paper that is 3-4 pages addressing the following questions. Use two or more scholarly sources including the assigned readings to develop this plan. Format the paper into sections based on questions below and use headers to organize.
What is the health risk factors for this age group and for Mr. Robert’s specifically?
Develop a questionnaire using the Health belief model to assess this patient’ perceived state of health or risk of disease and attach as an appendix.
Describe a learning needs assessment process individualized to this patient.
Develop short-term and long-term learning goals specific to this patient that address Mr. Robert’s health risks and include clinical goal.
Describe teaching and learning strategies you would use in our plan an encourages the patient to become an active learner.
Describe how technology can be helpful in implementing your plan.
