Pharmacists’ Patient Care Process Reflection questions
ntroduction
In 2014, the Joint Commission of Pharmacy Practitioners (JCPP) published a resource document endeavoring to promote consistency in the discussion of how Pharmacists reduce adverse drug events, improve patient safety, and optimize medication use and health outcomes as part of patient-centered care. Today, Pharmacists collaborate, document, and communicate with other pharmacists, physicians, and other health care professionals in order to provide safe, effective, and coordinated care.
The Pharmacists’ Patient Care Process was developed using key documents on pharmaceutical care and medication therapy management. It describes our roles in five parts: collect, assess, plan, implement, and follow-up.
